Appointment slots are $25 each and can be paid for at the time of appointment.
We are not accepting new caseworkers at this time. Please check back in November 2013.
Once we issue a username and password you will be able to login to access the Furniture Request Form, which asks for demographic information on your client.
After submission of the Furniture Request Form you will be redirected to our appointment calendar to schedule an appointment.
Please Note: If you/your client fails to show for the appointment or fails to cancel with a 24 hour notice you/your agency will be charged a $25 NO SHOW fee. You will unable to schedule or attend any appointments until that fee is paid.
